Summary
The Care Manager is responsible to assess, coordinate, facilitate and negotiate services and resources for a designated patient population in order to achieve desired clinical and financial outcomes. Works in collaboration with the patient's healthcare team to move the patient through the continuum of care.
Minimum Qualifications:
Graduate of an approved school of nursing. BSN preferred. Current, valid North Carolina Registered Nurse license. Minimum of one (1) year experience as a Registered Nurse in an acute care hospital. Current BLS certification.
